- name and surname of the patient as in the national passport
- name and surname of the accompaining persone as in the national passport
- dates of birth of persons specified in the invitation
- passports numbers of persons specified in the invitation
- validity dates of the passports
- addresses of the persons specified in the invitation (according to registration)
