Modern methods of treating ear cancer: from early diagnosis to complex therapy

Ear cancer is a rare but dangerous cancer that affects the outer, middle or inner ear. German clinics use innovative diagnostic and treatment methods, including microsurgery, radiation therapy and reconstructive surgery.

Ear cancer is a malignant tumor that develops in the tissues of the auricle, ear canal, or middle ear. The most common:
✔ Squamous cell carcinoma (80% of cases)

✔ Melanoma (rare, but extremely dangerous)
✔ Adenocarcinoma (tumor of the glands of the auditory canal)

Risk groups:

  • Persons with a hereditary predisposition

Symptoms of ear cancer

Cancer of the outer ear (pinna)

  • Non-healing ulcer or lump
  • Peeling, itching, bleeding of the skin
  • The appearance of nodules or warty growths
  • Pain radiating to the temple or jaw

Cancer of the ear canal and middle ear

  • Hearing loss
  • Discharge (sometimes with blood)
  • Tinnitus
  • Dizziness (with damage to the inner ear)
  • Facial paralysis (in advanced stages)

Important! In the early stages, ear cancer can masquerade as otitis media or eczema, so long-term symptoms require in-depth diagnosis.

    Classification and stages of ear cancer

    • 1. Main types of ear cancer

      2. Stages (AJCC 8th edition)

      A. Auricle:

      • 0 (Tis): Only the top layer of skin
      • I (T1): ≤2cm, no cartilage
      • II (T2): >2 cm or with cartilage
      • III (T3): Bone/temporal region
      • IV (T4): Skull, brain, salivary glands

      B. Auditory canal/middle ear:

      • T1: Passage only
      • T2: Bone/soft tissue
      • T3: Middle ear
      • T4: Cranial nerves

      3. Metastases (N/M)

      • N0: Not in lymph nodes
      • N1: There are in the near ≤3 cm
      • M1: In other organs

      4. Groups of stages

      • I: T1N0
      • II: T2N0
      • III: T3 or N1
      • IV: T4/N2/M1

    Ear cancer treatment methods

    1. Surgical treatment

    • Laser microsurgery – removal of small tumors without damaging healthy tissue.
    • Resection of the auricle – in case of extensive damage, followed by reconstruction.
    • Tympanotomy – removal of a middle ear tumor while preserving hearing.
    • Radical mastoidectomy – for damage to the temporal bone.

    2. Radiation therapy (IMRT, proton therapy)

    • Destroys cancer cells after surgery.
    • Used for inoperable tumors.

    3. Chemotherapy and targeted therapy

    • Used for metastases.
    • Selected individually (based on molecular tests).

    4. Reconstructive surgery

    Restoration of the auricle and auditory functions using:

    • Patient’s own tissue
    • Biocompatible implants

    Survival prognosis

    • At stage 0–I – cure in 95% of cases.
    • At stages II–III – 60–80% survival rate with complex treatment.
    • At stage IV, therapy is aimed at prolonging life and improving its quality.
