Screening and primary diagnosis
- Extended colposcopy – examination of the cervix under magnification with the Lugol test and acetic acid
- PAP test (liquid cytology) – identification of atypical cells
- HPV testing – determination of oncogenic types of virus (16, 18, 31, 33, etc.)
In-depth diagnostics
- Cervical biopsy (puncture, conization) with histological examination
- Immunohistochemistry (determination of p16, Ki-67, PD-L1)
- Molecular genetic tests (for adenocarcinoma)
Visualization
- Ultrasound of the pelvic organs (transvaginal)
- MRI of the pelvis – assessment of tumor extent
- PET-CT – detection of metastases (if stage III-IV is suspected)

Classification and stages of cervical cancer
Tumor types:
- Squamous (80-90%)
- Adenocarcinoma (10-20%)
- Rare forms (clear cell, neuroendocrine)
Stages (FIGO 2018):
- I – Limited by the cervix
- IA1-IA2 – microinvasion
- IB1-IB3 – visible tumor (up to 4 cm or more)
- II – Spread beyond the neck:
- IIA – vagina
- IIB – parametrium
- III – Up to the walls of the pelvis/lower third of the vagina + damage to the lymph nodes
- IV – Organ metastases (IVB)
Grade of malignancy:
- G1 (low) → G3 (high aggressiveness)
Key prognosis factors:
- Tumor size
- Lymph node involvement
- HPV/PD-L1 status
Diagnostics: colposcopy, MRI, PET-CT.
Treatment methods for cervical cancer
1. Organ-preserving operations (for stages IA-IB1)
- Radical trachelectomy (fertility preservation)
- Robot-assisted surgery Da Vinci
- Conization with cryotherapy (for pre-invasive cancer)
2. Combination therapy (stages IB2-III)
- Chemoradiation therapy (cisplatin + external beam irradiation)
- Brachytherapy with 3D planning
- Neoadjuvant chemotherapy (to shrink the tumor)
3. Immunotherapy of metastatic forms
- Pembrolizumab (for PD-L1+)
- Tisilelumab (in clinical trials)
Prevention and vaccination
- Vaccine Gardasil 9 (protection against 9 oncogenic types of HPV)
- Screening program:
- Up to 30 years: PAP test every 3 years
- After 30: PAP test + HPV typing every 5 years
- Rapid tests for HPV for home use
Survival prognosis
5-year survival by stage (FIGO 2018):
- Stage 0 (in situ): ~100%
- Stage I: 80-95%
- Stage II: 50-75%
- Stage III: 30-40%
- IV stage: 15-20%
Key prognosis factors:
- Early diagnosis (before cervical spread)
- No lymph node involvement
- Tumor size <4 cm
- Squamous cell type (better than adenocarcinoma)
