Treatment of pulmonary heart failure in Germany
Pulmonary heart failure: from pathogenesis to complex treatment

Pulmonary heart failure (CHF) is a clinical syndrome characterized by dysfunction of the right side of the heart due to increased pressure in the pulmonary circulation. This condition develops as a complication of chronic lung diseases (COPD, fibrosis, pulmonary embolism) and leads to progressive heart failure.
RheinRegio Klinik Zentrum (Germany) uses innovative methods of diagnosis and complex treatment of LHF, including modern drug protocols and surgical interventions to restore cardiopulmonary function.
Symptoms of pulmonary heart failure
The clinical picture combines signs of pulmonary and cardiac pathology:
Respiratory disorders
- Progressive shortness of breath (first with exertion, then at rest)
- Cyanosis (blueness of lips, fingertips)
- Cough with sputum
- Wheezing on auscultation of the lungs
Cardiac manifestations
- Tachycardia (rapid heartbeat)
- Edema of the lower extremities
- Swelling of the neck veins
- Liver enlargement
- Ascites (in later stages)
NYHA severity criteria:
- Stage I: Symptoms only with significant exertion
- Stage II: Shortness of breath with normal activities
- Stage III: Symptoms at rest, severe swelling
- Stage IV: Severe failure with multiple organ dysfunction

Diagnostics pulmonary heart failure
For accurate diagnosis the following are used:
Instrumental methods
- Echocardiography – assessment of pulmonary artery pressure and right ventricular function
- CT scan of the lungs – detection of primary pulmonary pathology
- Right Heart Catheterization – Accurate Measurement of Pulmonary Pressure
- Spirometry – assessment of respiratory function
Laboratory tests
- Blood gases (hypoxemia, hypercapnia)
- NT-proBNP (heart failure marker)
- D-dimer (exclusion of PE)
Differential diagnosis
Excludes:
- Coronary heart disease
- Cardiomyopathies
- Constrictive pericarditis
Treatment methods pulmonary heart failure
Drug therapy
- Diuretics – reduction of edema (furosemide)
- Oxygen therapy – correction of hypoxia
- Bronchodilators – improve pulmonary ventilation
- Anticoagulants – prevention of thrombosis
- Specific drugs (sildenafil, bosentan) to reduce pulmonary pressure
Surgical treatment
- Thrombendarterectomy – for chronic pulmonary embolism
- Balloon pulmonary angioplasty
- End stage lung transplantation
Rehabilitation
- Breathing exercises
- Physiotherapy
- Individual exercise therapy programs
Benefits of treatment in Germany
