Treatment esophageal cancer in Germany
An integrated approach to esophageal cancer: from endoscopic treatment to reconstructive surgery

Esophageal cancer is the 7th leading cause of cancer mortality. RheinRegio Klinik Zentrum uses innovative protocols combining neoadjuvant chemoradiotherapy, robotic surgery and modern methods of restoring esophageal patency. Our clinic specializes in the treatment of both squamous cell carcinoma and adenocarcinoma of the esophagus.
Symptoms of esophageal cancer
Early manifestations
- Dysphagia (first difficulty swallowing solid food, then liquid)
- Odynophagia (painful swallowing)
- Unexplained weight loss (more than 10% of body weight in 3 months)
Late signs
- Regurgitation of food
- Hoarseness (recurrent nerve damage)
- Cough when swallowing (tracheoesophageal fistula)
- Hemoptysis
Important: In 40% of patients, the disease is diagnosed at stages III-IV due to its asymptomatic onset.
Diagnosis of esophageal cancer at RheinRegio Klinik Zentrum
At RheinRegio Klinik Zentrum we carry out:
- Esophagogastroscopy with NBI and chromoendoscopy + biopsy
- Endoscopic ultrasound (assessment of depth of invasion)
- Whole body PET-CT (detection of distant metastases)
- Thoracoscopy/laparoscopy for precise staging
- Molecular genetic tests:
- HER2 status (for targeted therapy)
- PD-L1 (for immunotherapy)
Innovation: Confocal laser endomicroscopy for intraoperative assessment of resection margins.
Each direction is supervised by advanced oncologists.

Classification and stages of esophageal cancer
Histological types
- Squamous cell carcinoma (60% of cases) – associated with smoking and alcohol
- Adenocarcinoma (35%) – develops against the background of Barrett’s esophagus
- Rare forms: small cell, carcinosarcoma
Stages according to the TNM system (8th edition)
- T1a: Tumor within the mucosa
- T4b: Invasion into the aorta or spine
- N3: Metastases in 7+ regional lymph nodes
- M1: Distant metastases (liver, lungs)
Treatment methods for esophageal cancer
1. Surgical treatment
- Endoscopic mucosal resection (ESD) – for T1a
- Robotic esophagectomy (Da Vinci) – minimal trauma
- Reconstruction with a gastric or intestinal graft
2. Combination therapy
- Neoadjuvant chemoradiotherapy (CROSS protocol)
- Adjuvant immunotherapy (nivolumab for residual disease)
3. Palliative methods
- Esophageal stenting
- Photodynamic therapy
- Laser recanalization
Survival prognosis
- Early stages (T1): 5-year survival rate 80-85%
- Locally widespread forms: 30-40% after complex treatment
- Metastatic cancer: median survival 10-12 months
Factors for improving prognosis:
- Complete pathomorphosis after neoadjuvant therapy
- R0 resection
- HER2 positivity (response to trastuzumab)
Why choose Germany?
- Leader in esophageal surgery – 15% lower anastomotic leak rate
- Access to the latest drugs – the first in Europe to use a combination of nivolumab with chemotherapy
