Glioblastoma (GBM) is the most aggressive primary brain tumor (WHO grade IV). At RheinRegio Klinik Zentrum, combined treatment protocols are used, including intraoperative fluorescence, proton therapy and targeted drugs. Our goal is to maximize life extension while maintaining its quality.

Symptoms of glioblastoma

General cerebral manifestations

  • Headache (worsens in the morning, not relieved by analgesics)
  • Nausea and vomiting without relief
  • Seizures (in 50% of cases)

Focal symptoms

  • Paresis of the limbs (if localized in the motor cortex)
  • Speech disorders (temporal lobe tumors)
  • Visual hallucinations (occipital lobe damage)

Critical sign: Rapid increase in symptoms over 2-3 months.

Diagnosis of glioblastoma at RheinRegio Klinik Zentrum

  • MRI with contrast (typical picture of “rings with jagged edges”)
  • Perfusion MRI (assessing blood flow in the tumor)
  • PET with methionine (determination of infiltration boundaries)
  • Stereotactic biopsy with histology and:
    • IDH1/2 mutation status
    • MGMT methylation (prognostic factor)
  • Liquid biopsy (ctDNA analysis)

    Treatment methods for glioblastoma

    1. Neurosurgery

    • Craniotomy with 5-ALA fluorescence (complete resection in 80% of cases)
    • Intraoperative MRI (radicality control)

    2. Radiation therapy

    • Proton therapy (minimal damage to healthy tissue)
    • Stereotactic radiosurgery (for relapses)

    3. Systemic therapy

    • Temozolomide (standard for methylated MGMT)
    • Bevacizumab (for pseudoprogression)
