Hemangiopericytoma is a rare vascular tumor arising from pericytes (cells surrounding capillaries). It can be benign or malignant and often recurs and metastasizes. Features:
✔ Most often occurs in soft tissues (extremities, retroperitoneum)
✔ In 15–30% of cases it is localized in the central nervous system (meninges)
✔ Can be benign (low-grade malignancy) or aggressive (with metastases to the lungs, bones)
✔ In 40% of cases it recurs after removal

Symptoms of hemangiopericytoma

Manifestations depend on localization:

1. Soft tissue hemangiopericytoma

  • Painless, firm formation under the skin
  • Swelling and dysfunction of the limb (for large sizes)
  • Blueness of the skin (due to the vascular component of the tumor)

2. Hemangiopericytoma of the brain

  • Headaches (worsen in the morning)
  • Nausea, vomiting
  • Convulsions
  • Focal neurological symptoms (paresis, speech impairment)

3. Hemangiopericytoma of the lungs or abdominal cavity

  • Shortness of breath (with lung damage)
  • Intestinal obstruction (with retroperitoneal localization)

Diagnosis of hemangiopericytoma at RheinRegio Klinik Zentrum

  • MRI with contrast is the gold standard for brain and soft tissue tumors
  • CT scan of the chest/abdomen – search for metastases
  • Biopsy with immunohistochemistry (markers: CD34, STAT6, bcl-2)
  • PET-CT – assessment of tumor metabolic activity

    Classification and stages of hemangiopericytoma

    1. Low degree of malignancy

    • Growth: Slow
    • Mitoses: <1 for 10 fields of view
    • Necrosis: None
    • Metastases: Rarely

    2. High degree of malignancy

    • Growth: Fast
    • Mitoses: >4 for 10 fields of view
    • Necrosis: Present
    • Metastases: Up to 50% of cases

    1. Surgery

    • Radical resection involving healthy tissue is the main method
    • Neuronavigation and intraoperative MRI for brain tumors

    2. Radiation therapy

    • Stereotactic radiosurgery (CyberKnife) for inoperable cases
    • Proton therapy for relapses

    3. Targeted therapy

    • Sunetinib (angiogenesis inhibitor)
    • Bevacizumab for metastases

    Survival prognosis

    • Localized forms – 80–90%
    • Metastatic form – 45–60%
