    Classification and stages of prostate cancer

    Classification according to the TNM system and Gleason score:

    • Stage I – microscopic tumor, not palpable.
    • Stage II – the tumor is limited to the prostate, but is visible on ultrasound.
    • Stage III – cancer extends beyond the prostate capsule.
    • Stage IV – metastases to lymph nodes, bones, lungs.

    The earlier cancer is detected, the higher the effectiveness of treatment.

    Prostate cancer treatment methods

    Surgical treatment

    • Da Vinci robotic prostatectomy is a low-traumatic removal of the prostate while preserving the nerves.
    • Laparoscopic surgery – minimal incisions, quick recovery.

    Radiation therapy

    • Brachytherapy is the injection of radioactive grains directly into the tumor.
    • IMRT is high-precision radiation with minimal impact on healthy tissue.

    Drug therapy

    • Hormonal therapy – blocking testosterone, which feeds the tumor.
    • Targeted and immunotherapy – for resistant forms of cancer.

    Alternative Methods

    • FUS ablation (HIFU) – ultrasonic destruction of the tumor.
    • Cryotherapy is the freezing of cancer cells.

    Survival prognosis

    5-year survival rate:

    • Localized cancer (I–II stage): ~100%
    • Locally distributed (III stage): ~95%
    • Metastatic (IV stage): ~30%

    Key factors:

    • PSA level (<10 ng/ml – better prognosis)
    • Gleason sum (≤6 – low risk, ≥8 – high risk)
    • No metastases to bones/lymph nodes
    • Response to hormone therapy (in advanced forms)

    Note: Early stages are often treatable. Even with metastases, modern therapy (PARP inhibitors, radiation therapy) prolongs life by years.
