What is PSMA therapy?

PSMA (prostate-specific membrane antigen) is a protein that is produced in large quantities by prostate cancer cells in 90% of cases. The method is based on targeted effects:

  • The radiopharmaceutical (¹⁷⁷Lu-PSMA) binds to PSMA antigens.
  • Lutetium-177 beta radiation destroys tumors and metastases.

Indications for PSMA therapy

Contraindications:

  • Severe renal failure.

    Diagnosis before PSMA therapy

    At RheinRegio Klinik Zentrum we carry out:

    • PSMA-PET/CT – detection of foci with high antigen expression.
    • Biopsy + histology – confirmation of adenocarcinoma.
    • Blood tests (PSA, creatinine, hemoglobin).
    • Bone scintigraphy – assessment of bone metastases.

    How is the treatment carried out?

    • Preparation:
      • Discontinuation of certain medications (eg, bisphosphonates).
      • Hydration (to protect the kidneys).
    • Administration of the drug:
      • Intravenous infusion of ¹⁷⁷Lu-PSMA (5-7 minutes).
      • Course: 4-6 cycles with an interval of 6-8 weeks.
    • Monitoring:
      • PSA monitoring, MRI/PET-CT after 2-3 cycles.

    Side effects (usually mild):

    • Dry mouth, fatigue.
    • Temporary decrease in platelet/leukocyte levels.

    • A decrease in PSA by 50% or more – in 60-70% of patients.
    • Reduction of metastases – in 40-50% of cases.
    • Improved quality of life – reduced pain, increased activity.
