Treatment of CLL (chronic lymphocytic leukemia) in Germany
Personalized approach to therapy: from observation to innovative drugs

Chronic lymphocytic leukemia (CLL) is the most common type of leukemia in adults, characterized by the accumulation of mature B lymphocytes. RheinRegio Klinik Zentrum uses modern treatment standards based on the molecular genetic characteristics of the disease. Our strategy is to maximize disease control while maintaining quality of life.
Symptoms of CLL (chronic lymphocytic leukemia)
Early stages (often asymptomatic)
- Enlarged lymph nodes (cervical, axillary)
- Increased fatigue
- Night sweats
Progressive forms
- Hepatosplenomegaly
- Frequent infections (due to immunodeficiency)
- Anemia (pallor, weakness)
- Thrombocytopenia (bleeding)
Important: In 40% of patients, CLL is detected by chance through a blood test.
Diagnosis of CLL at RheinRegio Klinik Zentrum
At RheinRegio Klinik Zentrum we carry out:
- Detailed blood test (lymphocytosis >5×10⁹/l)
- Immunophenotyping (CD5+, CD19+, CD23+)
- Cytogenetic studies:
- FISH (detection of deletions 11q, 13q, 17p)
- Molecular tests:
- IGHV mutational status
- TP53 analysis
Innovation: Monitoring of minimal residual disease (MRD).
Each direction is supervised by advanced oncologists.

Classification and stages of CLL
Clinical stages (Rai/Binet)
- 0: Lymphocytosis without other symptoms
- II: Lymphocytosis + enlarged liver/spleen
- IV: Anemia + thrombocytopenia
Prognostic factors
- IGHV mutations (favorable prognosis)
- Deletion 17p (unfavorable)
CLL Treatment Methods
1. Active surveillance
- For asymptomatic stages 0-I
2. Drug therapy
- Combinations:
- Obinutuzumab + chlorambucil (1 line)
- Ibrutinib + venetoclax (at 17p-)
- Innovative drugs:
- BTK inhibitors (acalabrutinib)
3. Bone marrow transplantation
- Allogeneic (for refractory forms)
Survival prognosis
- Early stages: >80%
- With 17p deletion: 30-40%
Conclusion: Our clinic uses international protocols, taking into account the individual characteristics of the disease.
